Disease Management Programs:  Asthma

Adult and Pediatric Asthma Management Program

At HNE, we understand how difficult it can be to manage your asthma.  We also know that having asthma can interfere with your full enjoyment of life.  It may cause you to miss days from work or school and limit your activity.  That is why HNE has developed the Asthma Management Program.

This program is designed for adults with asthma or the parents of children with asthma.  Through a series of 4 coaching sessions over a 6 month period, the program staff works with you to determine your education needs and struggles.  You will work with your case management to develop control over your asthma and a strong relationship with your physicians.  The program is available to you free of charge.  As part of the program you will receive:

Educational Materials:  These materials are sent directly to your home.  All HNE members with asthma are eligible to receive written educational materials specific to managing their asthma.

If you would like to enroll in our 6 month program, please call 1-800-842-4464 and ask a Member Services Associate to connect you with the Care Manager for you.
