MBA administers groups with between 1-5 eligible employees. In order to obtain a quote, please forward the following to MBA:

• Name & Address of Group
• Full Census (include gender, coverage type, dates of birth)

A quote will be sent to you within 24 hours with an Enrollment Package. MBA will handle the implementation of the group, membership and commission payments.
