Q: Who will be receiving MA 1099-HC form?
HNE will send the MA 1099-HC form to Massachusetts residents who were enrolled as the subscriber on HNE on 12/31/07. The MA 1099-HC form sent to the Subscriber also lists any dependents (spouses, spousal equivalents, and children) over the age of 18 enrolled on the plan on 12/31/07.

Q: What if I was enrolled on HNE in 2007, but my coverage ended before
December 31, 2007?
The Ma Healthcare Reform law requires all Massachusetts residents to have health insurance on 12/31/07. Based on this, HNE will issue MA 1099-HC forms only to members whose coverage was effective on that date, If you did not have coverage on 12/31/07, you may lose your personal income tax exemption. If you were covered by another insurance plan on 12/31/2007, and you have not received a MA 1099-HC form, you should contact that plan directly.

The 1099 forms will be postmarked no later than 1/31/08.

Q: What information do I need to file my taxes?
You will need your HNE policy identification number (located on your ID card). If you do not have your MA 1099-HC form when you file, you may need HNE’s tax identification number. The tax identification number you should use depends on the group through which you enrolled. Look for the Group number on your ID card.
- If the Group number begins with the letter S, then use the tax ID number 043012347.
- If your Group number begins with any other letter or number, use the tax ID number: 042864973.
