The survey was developed at Dartmouth Medical School. It is completely confidential and anonymous.

Taking the survey.

• Once you get to the Web site, click “Step One: Begin Your Health Check-up.”
  
  
• Click “Click here if you do not have a code but would like to do the survey.”
  
  
• You and your family members can complete the survey by selecting the appropriate age and sex. All of your answers are confidential - no name identification is required.

• Based on your answers, you will be given special information personalized to your needs.
  
  
• When you’re done, you will receive a summary action form with your results. Please share this with your doctor or nurse.
