| The Sisters of Providence Health System Employer-Sponsored Health Plan includes an Exclusive Provider Organization (EPO) Plan. With an EPO Plan, In-Network services are provided by a limited network of contracted providers. Members in the Sisters of Providence EPO Health Plan must receive inpatient and outpatient services exclusively at Sisters of Providence Health System (SPHS) facilities.
SPHS and HNE have directed members enrolled in this Plan to work with their providers to ensure that services are received at Sisters of Providence facilities. Services that EPO members receive outside of the SPHS EPO network are treated as out-of-network services. HNE does not cover these services without prior approval. HNE has directed SPHS EPO Providers to follow HNE prior approval procedures when requesting care outside the Sisters of Providence network.
HNE has issued ID cards to members enrolled in this plan that identify them as SPHS EPO members. These ID cards include the Sisters of Providence Health System logo.
